OUR FEES
In providing our services, we believe in being transparent about pricing. Currently we accept self-pay patients and those who are insured by the following: Aetna, Anthem, BlueCross BlueShield, United, Cigna, Oscar, and Oxford. For patients who are self-pay, pricing is based on the type of visit. For our patients with insurance, pricing is determined by the type of visit and your insurance company. Other requests such as emotional support letters, bariatric letters, FMLA, disability have additional fees unless otherwise noted.
For Self-Pay Patients
Initial Evaluation - $300
A session to discuss past and current history, address current concerns, and prescribe or manage psychotropic medications.
Bariatric Surgery Psychiatric Evaluation and Clearance Letter- $175
A session to better a patient's understanding, motivation, readiness, behavioral challenges, and emotional factors that may impact their adjustment and recovery from Bariatric surgery. Upon evaluation and assessment, if a patient is cleared, they will receive a clearance letter from our nurse practitioners for their provider at no additional cost.
Emotional Support Animal Assessment and Letter - $175
A session to assess clearance for an Emotional Support Animal. This assessment is only for established patients as we feel in order to best determine a need for an ESA we need to have a better understanding of your mental health history. If you are cleared for approval for an emotional support animal you will receive an ESA letter at no additional cost.
GeneSight Service Package - $150
Service includes coordinating acquisition of testing and interpretation. Package includes providing a copy of results to patient. The package does not include the cost of the test itself. Cost may vary by insurance plan and out of pocket pricing may vary as well. Please see provider for further details. Additional information on GeneSight testing can be found at www.genesight.com
Psychotherapy - $200
A session to provide therapy to help process through behavioral and emotional difficulties.
Follow Up - Comprehensive Medication Management - $150
A session to discuss in-depth how your current medications are working and decide what updates need to be made to your current regimen to better improve current functioning.
Follow Up - Medication Management - $75
A session to review how your medications are currently working and refill current medications.

Please contact us to find out what your estimated fees and copays will include. If you do have an insurance that is not listed, upon request we can generate a super-bill that you can provide to your insurance company for out-of-network reimbursement.

Upon request, we can provide current patients an electronic copy of their records.
Family and Medical Leave Act (FMLA) and Disability Paperwork - $25
If a current patient has been assessed and it is determined they are a candidate for FMLA or Disability, an additional fee will be added to fill out FMLA or Disability paperwork.
Miscellaneous Forms - TBD
For other forms, price will be determined on a case by case basis.
Upon request, we can provide current patients an electronic copy of their records.

What do you need to get started today?
In order to qualify for services through ZenDeva, you must be a Texas resident and utilize a Texas pharmacy. Additionally, you must have access to a camera whether it be on a computer or on your smart phone!
